Since April 2023, the NCLEX includes Next Generation (NGN) items built to measure clinical judgment — not recall. The flagship format is the unfolding case study: one client, a chart that grows, and six questions that walk the six steps of the Clinical Judgment Measurement Model (NCJMM). If the format feels foreign, that's fixable in an afternoon. Here's the anatomy — and a complete worked example.
NGN items use partial credit. Select-all items often use +/− scoring: a point for each correct selection, minus a point for each wrong one, floored at zero — which means guessing everything is a losing strategy. Rationale-scored items award points for correct pairs (condition + action). Case-study items may use simple 0/1 scoring per item. The practical lesson: select only what the cues support.
The chart: A 68-year-old, post-op day 1 after bowel surgery. New notes: temperature 38.9°C (102°F), heart rate 118, respiratory rate 24, BP 98/60. The wound dressing shows new bright-red drainage; the client says "something feels like it popped." Abdomen is distended and tender.
Item 1 — Recognize cues: which findings require follow-up? The fever, tachycardia, tachypnea, borderline hypotension, new bright-red wound drainage, and the "popped" sensation. (Routine post-op soreness and a request for ice chips do not.)
Item 2 — Analyze cues: these cluster toward wound dehiscence with possible bleeding and early sepsis physiology — not normal recovery.
Item 3 — Prioritize: the most urgent concern is dehiscence/evisceration risk with hemorrhage — assess the wound now.
Item 4 — Generate solutions: cover any protruding contents with sterile saline-moistened dressings, keep the client NPO and supine with knees flexed, notify the surgeon, establish IV access, monitor vitals.
Item 5 — Take action (first): assess/cover the wound and call for help — don't leave to chart, don't push contents back in. Never reinsert protruding organs.
Item 6 — Evaluate: after intervention, reassess vitals and drainage; a stabilizing BP and controlled drainage indicate the immediate measures are working while awaiting surgery.
The Core Bundle includes full six-item case studies in every module — post-op deterioration, digoxin toxicity, DKA, preeclampsia, dehydration, a suicidal client, a deteriorating assignment — each with the reasoning written out the way this example is.
Next Gen NCLEX questions are where exams are won and lost. Our free field guide walks through every NGN item type — case study, bow-tie, trend, cloze, matrix — with fully worked examples and the scoring explained in plain English.
